Understand and practice all aspects of the role of the CNA/HCS within the parameters of the following rules, guidelines and regulations, the CNA/HCS will demonstrate their work listed below. The CNA/HCS apprentice will:

1. Abide by the laws of the Commonwealth of Pennsylvania and other governing bodies including the Health Insurance Portability and Accountability Act (HIPAA)
2. Comply with the policies and procedures established by TRIPL COMMUNITY SERVICES (TRIPL) in their handbook, and other processes as established in the workplace
3. Reflect the Independent Living Philosophy while assisting with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).
4. Perform services specified in the consumer’s information sheet. This means that the CNA/HCS will ask the consumer for direction in all activities and perform those activities as directed by the consumer and the consumer’s HHP
5. Provide competent personal care and homemaking tasks in the consumer’s home as assigned by TRIPL staff and consumer

1. Maintain confidentiality of consumer home and medical information
2. Demonstrate an awareness and respect of consumer’s rights and privacy
3. Perform the tasks on the consumer information sheet, while requesting and taking direction from the consumer and HHP, according to PA DOH Guidance to Home Care Agencies (Feb 2017) and the tenets of Nurse Delegation in Pennsylvania.
4. Respond immediately to emergency needs of consumer
5. Implement safety check of home environment and take precautions to minimize risk of accidents
6. Assist with and monitor, mobility, medical, safety, and therapeutic equipment and devices.
7. Demonstrate safe use of all home appliances, checking equipment-before use.
8. Demonstrate infection control practices and adherence to OSHA and CDC guidelines to minimize infection
9. Follow clean techniques.
10. Demonstrate consistent and appropriate hand-washing techniques
11. Perform other like duties to promote the health, safety and welfare as directed by the consumer and HHP

1. Treat consumer with respect, utilize person centered communication techniques, and demonstrate cultural competence, all within the Independent Living framework.
2. Utilize appropriate communication devices (communication board, etc.) with consumer
3. Explain to consumer what they will be experiencing, before and after care/assistive activities
4. Compile and disseminate, in accordance with HIPAA and TRPIL policy, correspondence or reports - to be made both verbally and in writing - of problems encountered, suggestions to the consumer's care, health changes noticed in consumer and other matters concerning the care, health, safety and welfare of the consumer.
5. Reports prepared are concise, easily understood, legible and include objective facts
6. Assist consumer as needed with personal/medical paperwork requirements
7. Maintain accurate records of time worked, distances traveled and any work related paperwork
8. Perform other like duties that assist consumer in communication as directed by the consumer and HHP

1. Encourage consumer self-care related to positioning, transferring and ambulation; including use of assistive devices/adaptations including prostheses. Assist consumers as necessary.
2. While assisting with ADL and IADL, demonstrates proper use of transfer equipment and other therapeutic or mobility devices as recommended the consumer and by HHP_ Employ handling techniques in a manner that makes the consumer feel safe.
3. Provide exercises as directed and appropriate
4. Implement turning and repositioning as well as other preventative measures to prevent skin breakdown, and promote healing of skin injuries
5. Assist in Work and Leisure Activities
6. Perform other like duties that promote health, function, and mobility as directed by the consumer and HHP

1. Prepare consumer for meals as needed; prepare, serve and remove food; feed consumer as needed
2. Distribute water and nourishment according to consumer's needs
3. Assist consumers with food and fluids, assuring correct texture/consistency; complying with diet recommendations as directed by the consumer and HHP; recording intake amounts if necessary
4. Demonstrate knowledge of assistive and nutritional support devices and provide assistance as directed by consumer as necessary and HHP.
5. Observe and report changes in eating habits or other nutritional observations
6. Perform other like duties that promote nutrition, hydration and health as directed by the consumer and HHP

1. Provide assistance according to the consumer information sheet as directed by the consumer
2. Assist with bathing, grooming, and hygiene tasks as specified in the consumer information sheet and directed by consumer and HHP, if applicable
3. Assure consumer is dressed in clothing that is clean, in good repair
4. Assists with application and care of therapeutic or assistive devices as directed by the consumer and HHP, if applicable
5. Assists with Toileting Activities, provides peri-care as needed, assist with bowel and bladder regimes and like duties utilizing clean technique, as specified in the consumer information sheet, and as directed by consumer and HHP
6. Measures and records accurately intake and output if necessary
7. Assisting consumers in maintaining bowel and bladder regimes, and similar tasks, as specified in the consumer information sheet, and as directed by the consumer and HHP
8. Perform other like duties that promote hygiene and health

1. Consistently and accurately takes and documents consumer assessment information consumer in a timely manner and reports as specified in the consumer information sheet and as recommended by HHP
2. Understand basic medical terminology, common diseases and conditions of body systems
3. Assure adequate levels of clean, functioning equipment and needed supplies for the consumer's home
4. Promptly performs errands as assigned.
5. Utilizes-observation skills and proper reporting of changes in consumers' mental and physical condition.

1. Perform consumer-specific, non-skilled care, that is specified on the consumer information sheet, is delegated by the HHP and directed by the consumer

1. Accompany consumer outside the home as directed by the consumer and by TRPIL COMMUNITY SERVICES when included in the service plan
2. Assist with household and other errands. Performs variety of miscellaneous duties as requested such as obtaining household supplies and running errands
3. Accompany consumer to activities outside of the home setting if instructed to do so in the consumer information sheet.

1. Provides assistance in palliative care as described on the consumer information sheet, directed by the consumer or consumer representative and Home Health Care Professional
2. Provides emotional support and uses therapeutic communication for those experiencing loss or grief
